"Self Portrait" - stereo vinyl 2LP set, CBS 66250 (NL), 1970, re-released 1975 and 1985:

All the sleeves and records were made in Holland. The records have plain inner sleeves. The 1970 sleeve was laminated and was printed in Holland. The 1975 and 1985 sleeves are not laminated and were printed in Holland by Shorepak, who made many of the UK sleeves. The 1970 records have orange CBS labels. The 1975 records have orange/yellow CBS labels The 1985 records have red CBS labels.
